- Scheduling a Laser Examination -

Individuals must first complete and return an application form to the NCLE in order to validate qualifications to sit for an examination. Once you are notified by the NCLE that you are so qualified, you have 90 dates from the date of the notice to schedule an examination with ETA International at a location convenient to you. ETA International will not schedule exams for individuals unless they have first been pre-qualified by the NCLE.

Individuals may schedule an examination in our Columbus Ohio facility at any time by calling 800-435-3131. Exams are given in the Laser Training Center of Professional Medical Education located at 4243 Diplomacy Drive, Columbus Ohio 43228. Go to the Laser Training Center page of their website to print a map of driving directions from downtown Columbus to the Training Center on the West side. Hotel options are also listed there if you need one.

Examinations are offered at the conclusion of courses  held by PME, and other qualified proctors at various locations around the country.
